Duration: 45 min Newborn/Infant assessment and treatment Price: $119.99 (includes HST) BEFORE YOUR FIRST VISIT An online health history intake form for your newborn/infant must be completed and submitted. Please note that the consent/waiver form must include a parent/guardian signature. Once your appointment is booked by the clinic, a link to this form will be sent to you by email. Initial Appointment Includes: • Detailed health history review • Answer any questions you may have • Comprehensive manual osteopathic assessment and treatment This initial assessment aims to create a personalized treatment plan tailored to your needs.
